Employee Benefits

Pooled Leave

Available for vacation or other personal matters. Employees with 1-5 years of service earn three (3) weeks each year or 4.33 hours per pay period. Employees starting their sixth year earn four (4) weeks each year or 5.77 hours per pay period.

Bereavement Leave

Up to three days of paid leave upon death of any members of the employee’s immediate family.

Medical Benefits

PPO medical plan through High Mark Blue Shield. Current deductible: $750 individual. $1,500 family. Employee contributions range from $18.83 – $54.89 per pay period, depending on covered family size.

Dental and Eye Benefits

Comprehensive plans at minimal cost (under $2.00) to the employee.

Short Term Disability

LHA pays 75% of the premium. Benefits begin on the 15th day of injury or illness. Benefit amount is 60% of employee earnings up to $1,500 per/week for a period of 26 weeks.

Long Term Disability

LHA pays 75% of the premium. Benefit amount is 60% of employee earnings up to $5,000 per/month. Benefits begin 180 days after disability.

Retirement Plan

LHA contributes 13% of each full-time employee’s annual earnings toward the retirement plan with no employee match. Eligibility begins at open enrollment following 90 days of employment.

457 Plan

Employees may contribute additional money into this pension plan. Eligibility begins at open enrollment following 90 days of employment.

Group Life and Accidental Death Insurance

Life insurance in the amount of (1) times annual salary up to $50,000. LCHA pays a portion of the premium.
